Cwm Taf Morgannwg University Health Board, NHS Wales, is recruiting for a Prison Mental Health post that extends primary care services for GPs and nurses, delivering an augmented mental health service for medium-term needs.

The post holder will independently assess and provide high-quality care, signpost to community services, and refer to the In Reach team within the prison. Welsh language skills are desirable and there is emphasis on multi-disciplinary collaboration.

Qualifications

• Independent assessment of mental health patients.
• Signposting to community services and referrals.
• Experience working in a prison or primary care setting is advantageous.

Responsabilités

• Assess and provide high quality care to patients with mental health problems.
• Signpost individuals to appropriate community services and refer to the In Reach team within the prison setting.
• Collaborate with multidisciplinary teams within the prison healthcare setting to educate and develop pathways of care.
