- Hours: 40 hours/week; day shift (8-hour shifts), every other weekend, 1 holiday
Job Description & Responsibilities
- Identify continuing care needs through assessment, interdisciplinary care rounds, and requests.
- Establish a coordinated discharge plan with patients, families, and community services based on transition needs.
- Coordinate continuing care services, including skilled nursing, durable medical equipment, and community services.
- Interface with aftercare providers to ensure smooth transition with the post-discharge plan of care.
- Conduct ongoing assessments to determine continued need for acute care and appropriateness/timeliness of treatments to optimize reimbursement.
- Collaborate with the Utilization Review team regarding level of care and length of stay.
- Coordinate continuing care for extended recovery, ED, and outpatient areas as requested.
- Document discharge plans, patient education needs, and care coordination accurately in the electronic medical record.
Qualifications & Requirements
- Current Massachusetts RN license.
- Bachelor's Degree in Nursing or higher healthcare degree preferred.
- CCM certification preferred.
- Minimum of 2 years of case management experience in an acute care setting.
- Ability to assess patient's admission and level of care needs (inpatient and observation).
- Strong interpersonal and negotiation skills with a positive, professional demeanor.
- Proficiency in Microsoft Outlook, Word, Excel, Epic, and Careport.
- Ability to work independently.
