Job description

Enhanced Care Team Leader

The enhanced care team offers additional support across all wards and departments for patients who may require extra assistance due to unfamiliar surroundings, medication issues, mental health concerns, or challenging behavioral needs.

Enhanced care is an essential part of a therapeutic plan, ensuring the safe and sensitive monitoring of patients' physical and psychological wellbeing while building therapeutic relationships. The team may support 'cohorted' patients or those requiring 1-1 support.

Responsibilities
  1. Manage the operational efficiency of the enhanced care team, including staff development, training, and working within the set budget.
  2. Provide professional leadership within the clinical area, offering direction and inspiration to multidisciplinary team members.
  3. Develop protocols, guidelines, and patient information materials.
  4. Collaborate with ward teams and the Liaison Psychiatry Service to deliver high-quality clinical support aligned with the principles of least restriction.
  5. Ensure systems are in place for planning, providing, and reviewing enhanced care services.
  6. Deliver therapeutic interventions to patients with mental health conditions, cognitive impairments, or psychological distress.
  7. Regularly report on team effectiveness, focusing on reducing restrictive practices and incidents of harm.
