Continuity of Care Team Lead - 12 months fixed-term opportunity, 37.5 hours

Are you a dynamic, passionate, and innovative midwife seeking the next step in your career?

An exciting opportunity has arisen for an enthusiastic midwife to lead our established case-loading team Blossom & Bloom. This team offers continuity of care to women and their families planning to birth in the community setting. While predominantly low-risk care will be provided, clients wishing to birth outside of guidance will be supported by the team within a multi-disciplinary approach.

You will be involved in the rollout of our CoC plan and liaise with Community Matrons and other CoC leads in developing three new teams next year. Your role will include management duties of an additional team once established.

This role involves cross-site working across Hampshire Hospitals. Our Trust provides maternity services across Hampshire, including Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital in Winchester, and Hampshire Hospitals Maternity Centre in Andover. The role is based at Andover Birth Centre, with travel required across sites.

Main Duties

You will need excellent leadership skills to support the team in delivering high-quality care and be open to embracing positive change and transformation to meet the needs of the local women. You will support women choosing to birth at home or in our free-standing birth centre, aligned with our plans to enhance continuity of care.

We seek an enthusiastic, confident clinical practitioner with experience in various midwifery settings, ideally within the community. Your ability to influence others will be key.

Person Specification

Education & Qualifications

• Registered Midwife with a first-level degree or equivalent, with evidence of CPD
• IT training or practice experience with hospital systems
• NIPE trained
• Master’s level education or first-line leadership course

Experience

• Minimum 3 years’ experience in an NHS healthcare setting
• Experience in audits, research, service improvement, and change management
• Management and leadership experience
• Experience leading a community team
• Experience working with multi-lingual populations
• Experience managing patient flow and site operations
