Job summary

Hywel Dda University Health Board is seeking an Enhanced Clinical Practitioner to deliver high-quality, evidence-based care for patients with undiagnosed or complex health needs across community and urgent care settings.

The post holder will work autonomously with senior support, contribute to governance and service development, and promote care closer to home while engaging in teaching and competency progression within the ECP framework.

Qualifications

Job overview

We are seeking a motivated and compassionate Enhanced Clinical Practitioner (ECP) to deliver high-quality, evidence-based care to patients with undiagnosed, evolving or complex health needs across a range of community and urgent care settings. Working within an agreed scope of practice and supported by senior clinicians, the post holder will undertake clinical assessment, examination, decision-making and management to provide safe, effective and person-centred care.

The successful candidate will work autonomously whilst recognising the limits of their competence, escalating appropriately when required and collaborating closely with multidisciplinary teams to ensure patients receive the right care, in the right place, at the right time. A key focus of the role is supporting timely intervention, admission avoidance and care closer to home.

The post holder will contribute to clinical triage, assessment, treatment planning, care coordination and service delivery across health facilities, patients' homes and virtual environments. They will play an active role in clinical governance, quality improvement, education and service development while progressing through the Health Board's ECP competency framework.

Main duties of the job

The post holder will undertake comprehensive clinical assessments, including history taking, physical examination, clinical reasoning and decision-making, to develop and implement appropriate treatment and management plans.

Working autonomously, with support from senior clinicians when required, to assess, triage and manage patients across a variety of settings, including healthcare facilities, patients' homes and virtual consultation environments. The post holder will ensure patients receive timely access to appropriate care, treatment or onward referral, while recognising the limits of their competence and escalating concerns as necessary.

A key responsibility is promoting care closer to home and ensuring patients receive the right care in the most appropriate setting.

The post holder will contribute to clinical governance, quality improvement, audit, service evaluation and service development activities, supporting the delivery of safe and effective care. They will also promote health education and self-management, participate in teaching and mentorship activities, and maintain a portfolio of evidence to demonstrate progression through the Health Board's Enhanced Clinical Practitioner competency framework.

The role demands excellent communication and teamwork skills, flexibility, professional accountability and a commitment to ongoing professional development to ensure the delivery of person-centred care across urgent, acute, community and integrated care services.

Qualifications and Knowledge
  • Registered practitioner (e.g NMC/HPC)
  • Evidence of relevant in-house training or Level 6 modules completed in patient assessment and clinical assessment or willingness to work towards
  • Evidence of maintaining a competency portfolio demonstrating achievement and progression in the knowledge, skills and clinical competencies required for the assessment and management of patients presenting within their scope of practice.
  • Knowledge of working under supervised clinical practice for assessing complex presentations, and evidencing development towards independent practice within their own scope of practice, including training, competency assessment and ongoing proficiency in the use of locally authorised patient group directions (PGD’s) where appropriate.
  • Engaging in training, reflective practice and ongoing development within enhanced clinical practice.
  • Completed, or willing to achieve within an agreed timeframe following appointment – Triage training, Verification of Death and DNACPR Training if applicable to service
  • Competent at phlebotomy / Cannulation / IV drug administration /ILS
  • Awareness of Clinical Governance
  • Awareness of topical issues in health and social care
  • Understanding of collaborative practice and the role of the enhanced clinical practitioner within the multidisciplinary team
