Job description

Within the context of an enablement approach and operating within an integrated service, the post holder will:

  1. Provide outcome-focused assessments to inform the provision and coordination of enabling packages of support.
  2. Commission and coordinate personalised, outcome-led support packages that promote independence.
  3. Work in partnership with service users, carers, family members, third sector, and health partners to deliver holistic support.
  4. Demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the service and the organisation.

This is a fixed-term position until 30/09/2026.

Requirements

This role involves regulated work with Children and/or Protected Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. The preferred candidate must join the PVG Scheme or undergo a PVG Scheme update check. If you have spent three or more months outside the UK in the last five years, an Overseas Criminal Record Check will be required. Employment is contingent upon satisfactory completion of these checks.

A minimum of one reference from your current or most recent employer is required.
