Job summary

Sefton Council is recruiting a motivated Social Worker to join the borough-wide Learning Disabilities & Autism (LD&A) team within Adult Social Care. You will deliver strength-based Care Act assessments, care planning and reviews for adults with learning disabilities and/or autism, to promote independence, wellbeing and positive outcomes.

In a supportive team, you will work with individuals, families and partner agencies, manage safeguarding and complex caseloads, and contribute to service

Qualifications

• Qualified Social Worker with Registration with Social Work England.
• Experience in Adult Social Care and working with learning disabilities/autism.
• Strong understanding of Care Act and related legislation.

Responsibilities

• Undertake strengths-based Care Act assessments, reviews and support planning.
• Manage a varied caseload including complex cases, safeguarding concerns and transitions into adulthood.
• Work in partnership with health colleagues and other professionals to achieve best outcomes.
• Support individuals and families to navigate services, including Continuing Healthcare (CHC) and Section 117 arrangements.
• Apply the Mental Capacity Act (MCA) in practice with best interests decisions and least restrictive options.
• Contribute to risk management and safeguarding processes.
• Participate in team discussions, supervision and service development.
