Target Housing is seeking a Housing Support Worker for the THRIVE Project in Sheffield. You will support individuals with complex needs, helping them build independence and secure stable tenancies through tailored, case-managed assistance.

Qualifications

• Experience supporting individuals with multiple complex needs.
• Ability to engage hard-to-reach clients and build trust.
• Strong risk assessment and support planning skills.
• IT literacy for case recording and reporting.

Responsibilities

• Deliver 1:1 and group-based support using a case management approach.
• Complete risk assessments, support plans, and safety plans.
• Motivate and empower clients to achieve tenancy independence.
• Design and deliver activity programmes focused on wellbeing and independence.
• Maintain accurate records in line with data protection standards.
