Job summary

Nova Wakefield District Limited is seeking a Live Well Wakefield Social Prescriber - Trailblazer to empower adults 18+ to improve health, wellbeing and employment. The role sits in the Trailblazer programme, bridging health support with local employment links across Wakefield, offering hybrid working arrangements and a 37.5-hour week.

You will create personalised care plans, collaborate with local services and support self-management sessions.

Qualifications

• Experience in supporting people to improve health, wellbeing and employment.
• Excellent listening, relationship-building and motivational skills.
• Ability to work independently and within multi-disciplinary teams.
• Committed to equality, inclusion and making support accessible to all.
• Strong understanding of employment issues and health inequalities.
• Excellent organisational, communication and IT skills.
• Full driving licence and access to a vehicle.

Responsibilities

• Support people one-to-one to identify needs, create personalised care support plans and address barriers that may affect wellbeing and employment.
• Work collaboratively with local VCSE organisations, health and social care, primary care and local employers.
• Support the delivery of self-management sessions.
