Job summary

Central London Community Healthcare NHS Trust seeks an experienced Enhanced Clinical Practitioner to deliver high-quality, unscheduled care in an urgent care setting. You will work autonomously in the Emergency Clinical Practitioner role, applying advanced diagnostic skills and clinical reasoning to manage acute, undifferentiated conditions.

You will contribute to professional development and work with a supportive team to maintain up-to-date evidence-based practice, while driving service

Qualifications

  • Registered Nurse with current NMC registration or Registered Health Professional (physio, paramedic) with HCPC.
  • Evidence of Minor Injuries Module at Level 6/7.

Responsibilities

  • Deliver care in collaboration with Emergency services and other health care professionals.
  • Define standards of care and practice to maintain or improve quality.
  • Work with senior staff on operational management and development of the service.
  • Act as team Leader on a rotational basis for day-to-day management.
  • Take designated responsibility for discrete areas of operational management/clinical practice.
  • Communicate complex information to patients, carers and staff.
  • Deliver care within a culturally sensitive framework.
  • Audit service performance and evaluate outcomes.

Are you an experienced Enhanced Clinical Practitioner (ECP) with a passion for delivering high quality care in an Urgent Care setting? We have an exciting opportunity for you to join our dynamic and supportive team in Central London. As a key member of our team, you will play a critical role in shaping the quality of care we provide. We pride ourselves on keeping our ACP/ECPs up to date with the latest evidenced-based practices, while also offering strong support for your professional development and growth.In this role, you will work autonomously as an Emergency Clinical Practitioner, providing expert care to patients with acute, undifferentiated conditions. Using your advanced diagnostic skills and clinical reasoning, you will deliver high quality, unscheduled care to those who need it most.

Main duties of the job
  • The post holder will deliver care in collaboration with Emergency services, other health care professionals, statutory and voluntary agencies.
  • To work in association with colleagues in defining standards of care and practice aimed at maintaining, developing or improving quality.
  • To work with the senior staff in the operational management and the organisational development of the service.
  • Act as the team Leader on a rotational basis assuming responsibility for day to day management of the service.
  • Take designated responsibility for discrete areas of operational management/clinical practice.
  • Communicate complex and sensitive information to patients, carers and other staff.
  • Deliver care within a culturally sensitive framework.
  • Implement, carry out disseminate and evaluate service audits

Education/Qualification
Essential
  • Registered Nurse with current NMC registration or Registered Health Professional (physio, paramedic) with HCPC.
  • Evidence of Minor Injuries Module at Level 6/7
Desirable
  • Evidence of Minor illness module at Level 6/7
  • Evidence of completion of Level 7 Advanced Clinical Assessment Module
  • Independent Prescriber (or on pathway to complete)
  • Qualification in prehospital care or community care
  • MSC in advanced clinical practice
Experience
Essential
  • Demonstrable competence in autonomous clinical assessment and management of patients, exercising professional judgement and delivering safe, evidence-based care within scope of practice
  • Evidence of consistently delivering high-quality, patient-centred care in accordance with professional standards and best practice
  • Experience in working autonomously in an emergency or other unscheduled care environment or working autonomously within a GP surgery.
  • Evidence of competence in assessing, managing and escalating care for adults and children at risk of deterioration, with demonstrable experience of timely referral to hospital-based services when clinically indicated
  • Experience of effectively working in collaboration, negotiating and liaising across organisational boundaries.
Desirable
  • Experience of team / service/ staff management.
  • Experience of implementing evidence-based care and developing others to do so
  • Experience of change management and implementing change in practice
Skills & Knowledge
Essential
  • Evidence of comprehensive range of clinical skills and expertise relevant to the role
  • Proven ability to prioritise, manage own workload and flexibility in doing so
  • Able to deal with conflict and manage appropriately
  • Proactive in identifying own and other's learning and development needs, and actively seek ways to meeting those needs
  • Specialist knowledge in clinical reasoning, history taking, physical examination, diagnostic skills and prescribing/treatment
  • Clinical knowledge across a range of long-term conditions.
Desirable
  • Able to lead and motivate a team and implement change
  • Knowledge of resource management.
