Job Summary

Northgate Surgery is seeking a compassionate, organized, and proactive Care Coordinator to join our team. This role is essential in supporting the health and wellbeing of our elderly, frail, housebound, and care home patients. You will work closely with GPs, nurses, social care teams, and community services to ensure patients receive coordinated, person-centred care that meets their individual needs.

Main duties of the job

Key Responsibilities:

  • Serve as the primary point of contact for elderly, frail, housebound, and care home patients and their families.
  • Coordinate and monitor care plans, ensuring timely follow-ups and reviews.
  • Liaise with multidisciplinary teams (MDTs) including GPs, district nurses, social workers, and community services.
  • Support proactive care planning, including frailty assessments, medication reviews, and advanced care planning.
  • Maintain accurate and up-to-date patient records using clinical systems (e.g., EMIS).
  • Identify and address gaps in care, including missed appointments, medication adherence, or social support needs.
  • Facilitate regular care home rounds and virtual MDT meetings.
  • Promote self-management and independence where appropriate, signposting to relevant services and support groups.
  • Assist with hospital discharge follow-ups and reduce avoidable admissions/readmissions.
  • Requirement to be free to visit care homes and housebound patients.

At Northgate Surgery we have 3 GP Partners, 1 Salaried GP, 1 ANP, 2 Practice Nurses, 1 HCA working alongside a supportive Reception/Administrative and Dispensary team.
