This is a hybrid role; successful candidates will be required to attend our Office as well as work flexibly across our Hubs. We are seeking experienced workers with knowledge of both pre and post 14 EHCP plans, and previous experience managing EHCP plans.

We are looking for dedicated and experienced Education, Health, and Care (EHC) Coordinators to join our SEND and Inclusion Service. The successful candidates will be responsible for coordinating and managing EHC plans and annual reviews for young people aged 0-25 with special educational needs and disabilities.

This role involves working closely with young people, their parents/carers, schools, further education colleges, training providers, and relevant professionals to develop high-quality, outcome-focused EHC Plans.

Key Responsibilities:
  1. Oversee the end-to-end process of EHCP maintenance, reviews, and amendments, ensuring compliance with statutory deadlines.
  2. Work collaboratively with families, schools, health and social care professionals to ensure EHCPs are tailored to meet the individual needs of children and young people.
  3. Ensure all EHCPs are developed and maintained in accordance with the Children and Families Act 2014 and the SEND Code of Practice (2015).
  4. Act as a key contact for families, providing guidance and support while ensuring a fair and transparent process.
  5. Maintain accurate records, track progress, and contribute to service development by identifying trends and areas for improvement.
