This is an exciting opportunity to be part of a team dedicated to putting people first by ensuring they receive the right support at the right time, including same-day discharge for those clinically ready to return home. As part of our Home Assessment team, you will collaborate with Health & Social Care and Acute colleagues to prevent delays in hospital discharge.
You will work with individuals, their families, and support networks to ensure quality support during the initial days after returning home. Our ambition within the North Lanarkshire Partnership extends beyond same-day hospital discharge to preventing admissions at the hospital front door and within the community, where crisis response can avert hospital stays. As the Home Support Team Leader, you will lead this new initiative.
Your responsibilities include operational oversight of community-based home support workers across North Lanarkshire, ensuring high standards of responsive and proactive care. You will assist the Service Delivery Manager in implementing systems of accountability, quality assurance, and performance management, including monitoring and reviewing personal support plans within statutory timescales. Managing a dispersed team of Home Support Workers requires excellent organization, management, and leadership skills to optimize resource use.
Extensive experience in social work, home support, voluntary, or independent sectors is essential. A SVQ Level 3 in Health & Social Care or equivalent is desirable; candidates without this qualification must complete it within three years of starting the role.
You must become a member of the Protecting Vulnerable Groups (PVG) Scheme before starting and be registered or eligible for registration with the Scottish Social Services Council (SSSC).
The post is based at Dalziel Building, with occasional work at other North Lanarkshire locations.
