Job description

Job Summary

Hounslow PCNs are seeking a Lead First Contact Paramedic to join Hounslow Health PCN, collaborating with other PCN Lead Paramedics. Our expanding multidisciplinary teams serve five PCNs in Hounslow: Brentworth, Chiswick, Feltham & Bedfont, Great West Road, and Hounslow Health.

Hounslow, a diverse outer London Borough with approximately 324,000 registered patients across 47 GP Practices, is committed to improving healthcare access and reducing health inequalities through various initiatives, including staff training, technological advancements, and partnerships with Hounslow Community Healthcare within West London NHS Trust.

We are looking for passionate individuals dedicated to enhancing care for residents in socioeconomically deprived areas, focusing on health equity and high-quality multidisciplinary general practice.

Main Duties
  1. Operational leadership alongside clinical directors to innovate and maximize PCN opportunities.
  2. Gain comprehensive experience in Primary Care across a PCN footprint, working closely with other ARRS roles.
  3. Collaborate with urgent and community services to shape and deliver community-based care, aiming to avoid hospital admissions and improve patient quality of life.
  4. Participate in service improvements, implement local initiatives, and contribute to leadership within the PCNs.
  5. Provide leadership, mentoring, and develop effective relationships with GP mentors.
  6. Travel is required to visit patients at home and to various PCN practices and colleagues across Hounslow, so personal transport is necessary.
