Job Description

The post holder will be an expert clinical practitioner collaborating with the Coronary Heart Disease Specialist Nurse team to deliver a nurse-led, evidence-based Secondary Prevention Service, aligned with patient needs, local guidelines, and the National Service Framework.

  1. Coordinate service delivery, development, and improvement.
  2. Act as an expert resource ensuring patients, relatives, carers, and staff have access to CHD nursing support and advice.
  3. Represent South Tees Hospitals NHS Foundation Trust, Community Division, with external agencies and develop policies across the Coast-to-Coast region.
All Staff Are Expected To
  • Undertake other tasks as required, including cross-cover within the department when necessary.
  • Adhere to and promote STHFT policies and procedures.

Responsibilities

  • Provide specialist assessment in CHD for primary and secondary care teams, influencing service strategy and policy.
  • Engage with external agencies to ensure seamless care delivery across interfaces.
  • Participate in clinical reflection, appraisal, and personal development.
  • Maintain up-to-date knowledge in CHD and related policies.
  • Act as a CHD expert in primary care, providing specialist care packages.
  • Enhance patient understanding and management of their condition.
  • Maintain effective communication networks with external agencies.
