About the Role

Your responsibilities as a Support Practitioner could include:

  1. Social Support: Assisting individuals to engage with their communities by planning and attending outings to leisure activities such as day centres, cinema, swimming, etc.
  2. Personal Care: Supporting people to maintain their wellbeing through assistance with bathing, dressing, arranging visits with healthcare professionals, incontinence care, etc.
  3. Practical Support: Helping people maintain their tenancy by encouraging daily household tasks such as housework, laundry, ironing, cooking, and shopping.
