Job summary

We have an exciting opportunity for a District Nurse Clinical Lead to join our caring and highly motivated Torressa Community nursing teams in the Saltash and Torpoint area of East Cornwall.

This is a key role within our Community Nursing service, supporting our mission to 'Delivering high quality care.' The role embraces our CARE values: Compassionate Services, Achieving High Standards, Respecting Individuals, and Empowering People.

Based in a geographically aligned Primary Care Area, this role will be clinically responsible for diagnosing, prescribing, planning, and evaluating care. The goal is to ensure high standards of care for patients and their families within the community. The post holder will work as part of Integrated Teams, collaborating across professional and organizational boundaries.

A significant aspect of this role involves monitoring the quality of care and leading service development initiatives.

We seek a dynamic, flexible, and compassionate practitioner with a broad range of competencies and adaptability to diverse clinical situations.

This is a community-based role requiring frequent travel across various locations in the county, including areas not accessible by public transport. Candidates must demonstrate how they will manage these travel requirements.

Main duties of the job
  • Monitor and improve the quality of care provision, leading service development initiatives.
  • Work as a registered nurse with or working towards the DNSPQ qualification.
  • Provide clinical leadership within an integrated team, managing community nursing caseloads proactively.
  • Ensure all nursing team members deliver evidence-based clinical care to the highest standards.
  • Collaborate with other health and social care agencies and professionals.
