Job summary

Main duties of the job

The post holder will work within the National Framework for Continuing Healthcare to deliver effective and equitable services in Sussex, supporting the CHC team across assessment, reviews, care planning, and case management. Experience in CHC is beneficial but not essential; relevant health or care experience and excellent communication skills are required.

As a Senior Practitioner, responsibilities include managing caseloads, quality assurance, statutory reviews, developing partnerships, conducting assessments, and supporting team development. Travel may be required.

Person Specification

Qualifications (Essential): Degree or equivalent in Nursing, HCPC, NMC, or Social Work England registration, and evidence of CPD.

Desirable skills: Negotiation, conflict management, operational involvement, problem-solving.

Skills and Knowledge (Essential): Patient assessment, workload management, communication, safeguarding, Mental Capacity Act, IT proficiency.

Desirable: Knowledge of NHS Continuing Healthcare policies, handling FOI and SAR requests.
