NHS Sussex ICB is seeking a Senior Practitioner to deliver effective services in Continuing Healthcare. The role involves managing caseloads, conducting assessments, and supporting team development. The post holder will work within the National Framework for Continuing Healthcare to deliver effective and equitable services in Sussex, supporting the CHC team across assessment, reviews, care planning, and case management. Experience in CHC is beneficial but not essential; relevant health or care experience and excellent communication skills are required.
As a Senior Practitioner, responsibilities include managing caseloads, quality assurance, statutory reviews, developing partnerships, conducting assessments, and supporting team development. Travel may be required.

Deliver high-quality services, support staff, manage caseloads, ensure quality assurance, undertake statutory reviews, develop partnerships, support assessments and reviews, manage referrals, handle enquiries, complete assessments, and support team training. Flexibility and travel are required.
Qualifications (Essential): Degree or equivalent in Nursing, HCPC, NMC, or Social Work England registration, and evidence of CPD.
Experience (Essential): Working in health or care environments, understanding of roles and responsibilities.
Desirable skills: Negotiation, conflict management, operational involvement, problem-solving.
Skills and Knowledge (Essential): Patient assessment, workload management, communication, safeguarding, Mental Capacity Act, IT proficiency.
Desirable: Knowledge of NHS Continuing Healthcare policies, handling FOI and SAR requests.
