Our client based in the North East is seeking an 18+ Leaving Care Personal Advisor to join their Leaving Care team. The role involves providing statutory social work services for children in care and care leavers aged 18-25 years, ensuring compliance with legislative requirements. The work includes case management for eligible, relevant, former relevant, and qualifying care leavers, involving coordination, preparation, and completion of pathway plans, and supporting young people in preparing for independent living.

Your primary responsibilities include:
  1. Acting as a full member of the service team and contributing to its development within the directorate.
  2. Managing an individual caseload, working with young people and groups, and participating in duty work as needed.
  3. Providing appropriate programs for service users in collaboration with other staff and agencies, focusing on transition to adulthood and independent living.
  4. Implementing and adhering to all relevant procedures as an aftercare adviser.
  5. Ensuring positive and sustainable outcomes through best practices, with a passion for supporting children and their families to thrive, while prioritizing protection and efficiency.
Ideal candidate will have:
  • Substantial experience working with children and young people up to age 21, or 25 if in higher education, in voluntary or professional settings.
  • Experience in interagency work with vulnerable, at-risk, homeless, or former care young people.
  • Experience responsible for direct assistance to young people (caseload responsibility).
  • Proven success in achieving outcomes for young people under supervision.
  • Proficiency in writing emails, reports, case records, and using electronic case recording systems and software like Word.
  • Ability to advise on budgeting and financial accountability where appropriate.
  • Ability to secure education, training, and employment opportunities for care leavers.
  • Strong communication skills, including the ability to develop good relationships with young people, families, carers, and professionals across various sectors.
  • Aspirational attitude towards young people's development.
  • Good verbal presentation skills, time management, flexibility, and commitment to anti-oppressive and anti-discriminatory practice.
