An exciting opportunity exists for a Band 5 Learning Disability Nurse/Care Co-ordinator to work within an established Integrated Team alongside an MDT including Social Workers, Nursing, Psychiatry, Psychology, Physiotherapy, Occupational Therapy, Speech and Language Therapy, and administrative staff.
Durham County Council leads the Integrated Learning Disability Teams. The candidate will primarily use the Durham County Council system Azeus for recording, with access to TEWV systems, training, and NHS mail.
The team collaborates to ensure high-quality care, supporting individuals with learning disabilities and their families, utilizing the Care Act and TEWV clinical pathways. The service operates from 9 am to 5 pm, Monday to Friday, with the post holder participating in an out-of-hours Nurse on Call rota from 8 pm to 8 am.
The successful candidate will have a genuine desire to improve mental health and well-being among adults with learning disabilities and challenging behaviors. They will work as part of an MDT delivering secondary care, using trauma-informed and recovery-based approaches.
Applicants must be able to travel independently and have access to a suitable vehicle. Adjustments can be considered under the Equality Act 2010.
