Job overview

You will be working as part of a multi-professional team managing a GP-aligned caseload of patients in the community, ensuring their needs are identified and met. Promoting patient independence and supporting their physical, psychological, and social wellbeing.

We collaborate with community, primary care, and social services teams to enhance patient experience through innovative solutions.

Main duties of the job
  1. Work as part of a community integrated service responsive to housebound patients within a defined caseload.
  2. Reduce inappropriate hospital admissions, minimize hospital stay lengths, and support vulnerable clients at home through comprehensive assessments and care plans in partnership with GPs and other health and social care professionals.
  3. Collaborate closely with GP surgeries, Clinical Commissioning Groups, primary and secondary care, social care professionals, voluntary agencies, and the London Ambulance Service.

Person specification
