Adults Social Worker

We are looking for an Adult's Social Worker to join the Older People and Physical Disabilities Team.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

This team is designed to help those who are over the age 65+, the team will be working with care homes, nursing homes, and supported living. This team deals with cases on a long term basis with a expectation to run around between 6 - 12 weeks. Key factors of this team will be to undertake face to face visits of those individuals within the community, practice under relevant legislation, review over care packages and update as and when required.

About you

The successful candidate will have previous experience of working with relevant legislation such as Care Act 2014, MCA, Section 42. A Social Work (Degree/DipSW/CQSW) within a minimum of 1 year post ASYE experience is required. Experience within Community, OP,PD, complex care, front door will be transferable for this post.
