About the Role
As a Community Support Worker, you will meet with individuals, families, and carers to connect, listen, and undertake assessments under the Care Act 2014. You will develop plans to help individuals access support through community connections, referrals, or direct services. You will identify and report safeguarding concerns, consider mental capacity, and handle high-risk situations to ensure safety and liberty. The role involves visits to clients' homes, hospitals, or other comfortable locations, with a team-based, hybrid working approach.

Home First is a frontline team providing short-term support to older adults and those with physical or mental health conditions. The role requires managing your case load efficiently, working at pace, and demonstrating assertiveness, resilience, and teamwork. Supportive training and induction are provided.

As a Grade 9 Community Support Worker, you should have experience and skills to manage your workload and deliver high-quality, person-centered, statutory social care, including assessments and decision-making.

A DBS enhanced check is required, and a valid full driving license or access to a vehicle is necessary.
