Qualifications

  • Experience in providing holistic and patient-centered care in a hospice setting.
  • Strong understanding of regulatory safeguarding and Mental Capacity guidance.

Job description

Job Summary

To work as a key member of the Wellbeing team, contributing to a co-ordinated and efficient service reflecting the aims and philosophy of hospice care.

To promote and maintain the highest quality of care to patients, their families, and significant others, working as part of multidisciplinary and partnership teams using initiative in planning patient care.

To be motivated, enthusiastic, and forward-thinking in developing a holistic, goal-focused service.

To contribute to a co-ordinated and efficient service, with an understanding of all departments, reflecting the aims and philosophy of hospice care.

Main duties of the job
  • In conjunction with the Wellbeing Services Senior Palliative Care Nurse, ensure prompt and accurate assessment of relevant newly referred patients for hospice services, applying a holistic and patient-centred approach.
  • Develop a programme of care for patients referred to the Wellbeing service, meeting their individual needs in liaison with the multidisciplinary team.
  • Discuss with patients regarding Advanced Care planning, ReSPECT, Lasting Power of Attorney, etc., to support patient decisions.
  • Supervise activity on a day-to-day basis and ensure efficient management of the Wellbeing service during duty hours.
  • Maintain high clinical standards and quality of care, contributing to a caring and friendly environment.
  • Possess a full understanding of regulatory safeguarding and Mental Capacity guidance, and work flexibly to meet the needs of a growing service.
