Successful candidates will be employed by one of the above PCNs and aligned to a neighbourhood-based Integrated Neighbourhood Team (INT). Please specify your preferred PCN(s) or neighbourhood(s) in your application. Final placement will depend on service needs and candidate suitability.
We are seeking dedicated and proactive Health and Wellbeing Coaches to join our Integrated Neighbourhood Teams in Lewisham.
The roles focus on delivering the Lifestyle Medicine Service within the PCNs, aiming to improve health outcomes through holistic and personalized coaching, especially in nutrition and lifestyle medicine.
The Lewisham Health and Care Partnership is committed to creating a sustainable health and care system that supports both physical and mental wellbeing. Our Integrated Neighbourhood Teams integrate NHS services, primary care, social care, and the voluntary sector to provide coordinated, community-based support.
Modality Lewisham provides patient-centered primary care across central and south Lewisham following the merger of several practices. We focus on accessible, high-quality services and personalized health support, upholding NHS values and improving outcomes for our diverse community.
Aplos Health PCN is a collaborative network of four GP practices in south and west Lewisham, serving over 50,000 patients. We deliver integrated, community-focused care, addressing health inequalities through innovative services such as youth wellbeing clinics and digital hubs. Our multidisciplinary team is dedicated to improving health outcomes and empowering local residents.
