This is a fixed-term opportunity for an advanced clinical pharmacist with leadership and clinical expertise to lead the mental health pharmacy team within West Lothian HSCP, based at St Johns Hospital. The role involves working across mental health specialties, including adult and older people's mental health, and participating in service delivery and development, including community mental health services and transfer of care.
Key responsibilities include providing pharmaceutical care to vulnerable patients, medication reviews, medicines information, high-dose antipsychotic monitoring, adherence to the Mental Health Act, and patient/carer education.
The NHS Lothian Primary Care Pharmacy and Medicines Service is undergoing transformation and integration, with a team of around 100 staff providing care across General Practices and Pharmacotherapy Hubs.
