This role involves working with the Haematology Malignant Team to deliver high-quality support to patients with cancer. The post requires daily contact with patients suffering from various blood cancers and those with suspected cancer. Responsibilities include supporting and signposting patients over the telephone and progressing their care pathways in coordination with colleagues and departments. The post also involves participation in audits and Quality Surveillance reviews.

Main duties of the job
  1. Work as part of the Haematology Malignant Team to coordinate and provide care for patients on the Haematology Malignant Pathway.
  2. Assist Clinical Nurse Specialists (CNS) in delivering high-quality patient care through ongoing assessments and proactive needs identification using basic clinical knowledge and approved tools.
  3. Collaborate with CNS and the wider healthcare team to ensure appropriate services are provided, aiming to maximize patient health and quality of life, and improve healthcare efficiency.
  4. Serve as the single point of contact for patients, carers, and the healthcare team, triaging inquiries to the appropriate departments and providing routine information and advice over the phone, liaising with CNS as needed.
  5. Document and monitor all aspects of patient care coordination and service delivery, supporting data collection for audits.
