Southern Health NHS Foundation Trust and Solent NHS Trust are seeking one or two Consultant Geriatricians to contribute to the development and delivery of community services in the Portsmouth and South East Hampshire Locality. The successful applicants will be based at Gosport War Memorial Hospital (GWM) and/or Petersfield Community Hospital (PCH), working across inpatient and community services as part of a multidisciplinary team (MDT).
A key aspect of the role involves providing medical advice and guidance to inpatient wards, offering senior clinical oversight to complex patients across four community hospital wards and two frailty virtual wards. While day-to-day ward beds management is handled by nurse consultants, Specialty Doctors, and Advanced Clinical Practitioners, the consultant geriatrician will facilitate the MDT in managing complex cases, frailty, and expanding step-up care capabilities for local patients. Collaboration with acute Trust colleagues will support appropriate treatment escalation and step-down rehabilitation pathways.
