Job description

Job Summary

Southern Health NHS Foundation Trust and Solent NHS Trust are seeking one or two Consultant Geriatricians to contribute to the development and delivery of community services in the Portsmouth and South East Hampshire Locality. The successful applicants will be based at Gosport War Memorial Hospital (GWM) and/or Petersfield Community Hospital (PCH), working across inpatient and community services as part of a multidisciplinary team (MDT).

A key aspect of the role involves providing medical advice and guidance to inpatient wards, offering senior clinical oversight to complex patients across four community hospital wards and two frailty virtual wards. While day-to-day ward beds management is handled by nurse consultants, Specialty Doctors, and Advanced Clinical Practitioners, the consultant geriatrician will facilitate the MDT in managing complex cases, frailty, and expanding step-up care capabilities for local patients. Collaboration with acute Trust colleagues will support appropriate treatment escalation and step-down rehabilitation pathways.

Main Duties

The role emphasizes clinical leadership on two levels:

  1. Clinical Support and Advice: Providing accessible clinical guidance to community teams (e.g., District Nurses, Community Matrons, Therapists) and GPs. The post requires a shift from direct patient care towards offering advisory, educational, and supportive clinical input to empower primary care and community services in managing complex and frail patients at home.
  2. Service Development and Transformation: Leading the adaptation of clinical service structures to support patients in their homes, including strengthening step-up care to community hospital wards and enhancing support for home-based patients. The post-holder should be comfortable with change and enthusiastic about leading service transformation, ensuring it is clinically informed and driven by expertise.
