Role Overview:

We are seeking a dynamic and experienced Early Intervention and Prevention (EIP) Partnership Manager to lead and support transformative change across the Kirklees health and social care system.

This role focuses on adult social care front door integration, Integrated Neighbourhood Teams, and Personalised Care.

Key Responsibilities:

  • Leadership & Collaboration: Work collaboratively with colleagues and partners to review current service delivery, identify challenges and areas of good practice, and enhance partnership working.
  • Service Improvement: Identify development areas to improve access to services, direct resources to vulnerable communities, and enhance outcomes for all service users.
  • Data-Driven Decisions: Establish systems for investment and disinvestment decisions based on data, intelligence, and understanding of needs across different cohorts and services.
  • Integration & Influence: Utilise learning from the recent integration of Kirklees adult social care front door to inform and influence future service delivery.

Requirements:

  • Extensive experience in health, social care, and community settings.
  • Strong understanding of personalised care approaches.
  • Proven experience in developing and implementing cross-system working and establishing effective partnerships.
  • Ability to gather and present complex data to inform decision-making and future service developments.
