We are seeking two experienced Early Help Practitioners to take on a Lead role with families. Responsibilities include completing timely and robust whole family assessments, developing family plans, and convening multi-agency team round the family (Taf) meetings to ensure holistic, planned, and focused family support in line with the principle of one family, one plan.
