Main duties of the job
The Early Facilitated Discharge Practitioner will support the Trust's Crisis Resolution and Home Treatment Team (CRHTT) working alongside the Discharge Pathway Team to enable timely discharges from hospital and to optimise discharge from hospital.
The process of discharge from acute mental health wards can be complex and stressful for patients and their support network. Early stages of discharge can be associated with increased risks to a person's safety and recovery. Despite these risks, going home as early as possible can empower individuals to continue their recovery in a less restrictive environment, whilst ensuring that they receive the necessary support, help people maintain important relationships, reduce the stigma they experience and is desired by many people experiencing acute distress.

Attend Ward Red-to-Green Rapid Handovers, MDTs, Professionals Meetings and Ward Patient Reviews to collaboratively identify patients who are appropriate for EFD.
Engage with the Acute Wards at an early stage in the patients journey evaluating progress against the purpose for admission, identifying any barriers and supporting care planning by being proactively involved in the discharge planning for patients who are likely to require support by CRHTT post discharge.
Attend patient reviews on the Wards to assess the individuals recovery and willingness to engage with CRHTT post discharge.
