Early Facilitated Discharge Practitioner - Band 5: [Subject to AFC Banding Review]

An exciting opportunity has arisen for a Early Facilitated Discharge Practitioner to work along side the Discharge Pathway Lead and Discharge Co-Ordinators to support the Trusts agenda to minimise delayed discharges.

The Early Facilitated Discharge Practitioner will support the Trust’s Crisis Resolution and Home Treatment Team (CRHTT) working alongside the Discharge Pathway Team to enable timely discharges from hospital and to optimise discharge from hospital.

The process of discharge from acute mental health wards can be complex and stressful for patients and their support network. Early stages of discharge can be associated with increased risks to a person’s safety and recovery. Despite these risks, going home as early as possible can empower individuals to continue their recovery in a less restrictive environment, whilst ensuring that they receive the necessary support, help people maintain important relationships, reduce the stigma they experience and is desired by many people experiencing acute distress.
