Job summary

Torbay Council is recruiting Social Workers to join our Adult Social Care service at an exciting time of transformation. The permanent role covers 37 hours and offers development across Transitions, Complex Care and Older Person's Mental Health.

You will work with adults to assess needs, develop person-centred care plans, collaborate with families and partner agencies, promote independence, and contribute to safeguarding with supportive supervision.

Qualifications

• Qualified Social Worker registered with Social Work England.
• Commitment to continued professional development.
• Ability to work with adults to promote independence.

Responsibilities

• Undertake strengths-based assessments and reviews.
• Work collaboratively with individuals, families, carers, and partner agencies.
• Develop person-centred care and support plans.
• Promote independence and positive outcomes.
• Contribute to safeguarding activity where required.
• Work as part of multidisciplinary teams to coordinate support and reduce risk.
