We are recruiting for a Specialist Pharmacist-Frailtywho shares our ambition and vision to deliver theBest of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.

Main duties of the job

The Frailty Directorate is undergoing as exciting expansion with the development of a Frailty 7-day Same-Day Emergency Care service and Rapid Access Care of the Elderly clinics. This role would be supporting the development of an Integrated Frailty Medicines Optimisation Pharmacy Network.

We as a team are keen to focus on continuous improvement of our services for the benefit of our patients and welcome new ideas and opportunities. We aim to continuously upskill and develop our workforce and currently pharmacy assistants on ward areas supporting with medicines management including dispensing on wards, and medication supply checks for discharges, improving safety and reducing waste.

Together there will be structures to expand the team, work on improving patient flow and optimising patient care through medicines management across sectors.
