An exciting opportunity has arisen for a Band 5 Community Nurse working within the Northwest Integrated Neighbourhood Team based at Hartismere Hospital in Eye. This is a 12-month fixed-term/secondment opportunity.
Please note: This could be a Band 6 position depending on relevant experience. This will be assessed at interview.
The team consists of district & community nurses, assistant practitioners, generic workers, occupational therapists, physiotherapists, and social care workers. It is led by a Nursing Clinical Lead, integrated with a Therapy Clinical Lead, and managed by an INT manager. You will work across the Northwest INT and wider community services to meet service requirements.
The team prides itself on a holistic approach to care and continues to evolve to support NHS developments, including close collaborations with local statutory, non-statutory, and voluntary services.
You will receive regular supervision, annual appraisal, and opportunities for skills development training relevant to the role. You are expected to work autonomously on behalf of the patients served in the local area.
Shifts are operational from 08:00 to 18:00, covering 7 days a week.
Due to the community nature of the role, applicants must hold a UK driving license and have access to transport from the start of employment.
Your responsibilities include wound care, leg ulcer management, pressure area care, palliative care, venepuncture, attending GP multidisciplinary meetings, PICC line care, catheterization, equipment ordering, mentoring students, and involvement in link roles such as tissue viability.
The team offers various training opportunities to enhance and maintain your clinical skills, including in-service training, professional forums, mandatory training, personal development meetings, and clinical skills training facilitated by practice development facilitators. We also support student placements and maintain close working relationships with neighbouring community teams in health and social care.
