Radis Community Care has an opportunity for a Band 6/7 Community Lead Nurse to join our team operating across the Central & West Midlands.

We care for a range of service users, including the elderly and people with physical and/or learning disabilities. Our care is tailored to meet individual needs and ranges from brief daily visits to 24-hour live-in care, suitable for both short and long-term support.
About the role
As a Community Lead Nurse, you will undertake care assessments and clinical risk assessments, providing advice and guidance to the wider team. You will be the clinical lead for complex packages within your area, leading and managing Support Workers, ensuring they are trained and up-to-date in both theory and practice, in line with specific client care needs. The role includes shared on-call responsibilities.
You will work as part of a multidisciplinary team including commissioners, GPs, family members, and community professionals. The successful candidate will ensure all safeguarding issues related to vulnerable adults are reported according to policies and procedures.
The role offers opportunities to develop teaching skills, participate in staff education, and create training profiles for Radis Complex Care clients. Responsibilities also include maintaining, reviewing, and auditing all client clinical documentation, both in the service and at the CQC/CSSIW Registered Office.
This role may involve some remote working opportunities.

We seek a self-motivated, professional Registered General Nurse with experience in delivering training, ideally with experience working with complex clients in a community setting. Excellent communication skills are essential, along with a willingness to pursue PTLLS qualification. The role involves some out-of-hours and overnight work as needed. Travel across a large geographical area is required, so a full driving license and access to a vehicle are essential.
