Job summary
Hillton PCN is seeking an experienced advanced nurse practitioner to join our team on a full or part-time basis.
The post-holder will be required to act within their professional boundaries, providing care for presenting patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice.
The post-holder will demonstrate critical thinking in clinical decision-making. They will work collaboratively with the general practice team to meet patients' needs, support policy and procedures, and provide nurse leadership as required. To work at this level, NMC requirements for advanced practice must be met. Whether you have developed a specific specialty or are interested in doing so, we will support your ongoing career and training development.
Serving approximately 33,000 patients across Ampthill and Barton-Le-Clay, Hillton PCN aims to develop collaborative working across three member practices to support primary care services aligned with the NHS long-term plan.
We strive for high standards of patient-centered care tailored to our population's needs, supporting a resilient and well-trained workforce. We operate SystmOne with a good record of QOF achievement and a 'good' CQC rating across all domains.
Our team includes a paramedic, three clinical pharmacists, a care coordinator, a social prescriber, two physiotherapists, and a PCN manager. We are recruiting for additional roles to support our future development.
