Job summary

Hillton PCN is seeking an experienced advanced nurse practitioner to join our team on a full or part-time basis.

The post-holder will be required to act within their professional boundaries, providing care for presenting patients from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice.

The post-holder will demonstrate critical thinking in clinical decision-making. They will work collaboratively with the general practice team to meet patients' needs, support policy and procedures, and provide nurse leadership as required. To work at this level, NMC requirements for advanced practice must be met. Whether you have developed a specific specialty or are interested in doing so, we will support your ongoing career and training development.

Main duties of the job

1. Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad, in-depth theoretical knowledge base.
2. Assessing, diagnosing, and treating patients' conditions, initiating direct referrals as appropriate.
3. Providing expert advice to patients, carers, and colleagues.
4. Developing innovative concepts, models, methods, and practices to improve primary care delivery.
5. Advancing patient health through health promotion, education, screening, and therapeutic communication skills.
6. As an independent prescriber: prescribing safe, effective, and appropriate medication, as defined by current legislation.
7. Leading and undertaking research and audits.
8. Providing leadership, education, and training to healthcare professionals, staff, and students.
9. Supporting and caring for a diverse patient group as part of our multidisciplinary team.
