Practice Nurse - Full-Time Permanent Role

We are currently recruiting on behalf of a well-organised GP Surgery in the Liverpool, Merseyside area. Are you looking for a fresh new opportunity, where you can be part of a forward-thinking team? Salary - Band 6 equivalent - DOE

Start Date: ASAP

Working Pattern: Full-time hours, up to 37.5 hours a week - can be done over 4 days - flexible to discussion

Hours: Any working hours between 7am - 8pm - the surgery is open for enhanced access clinics

Job Summary:

Working as part of a small team, you will work alongside the lead nurse and be responsible for agreed lead areas.

A forward-thinking practice and staff development are at the forefront of what they do, therefore are looking for an engaging team member who can also work autonomously and work within their enhanced primary care team.

Key Responsibilities include:

• Ensure Compliance and Adherence to Guidelines:

• Ensure all cytology programs adhere to national guidance and regulations.

• Team Collaboration and Quality Contracts:

• Work collaboratively within the team to ensure nursing participation in quality contracts, such as QOF/GP Spec.

• Patient Assessment and Evidence-Based Healthcare Provision:

• Assess patient needs to ensure the provision of healthcare is appropriate, incorporating evidence-based practices.

• Health Promotion and Wellbeing:

• Develop, implement, and embed health promotion and well-being programs.

• Chronic Disease Management:

• Implement and evaluate individual treatment plans for chronic disease patients, identifying, managing, and supporting those at risk of developing long-term conditions.

• Clinical Responsibilities and Patient Support:

• Provide routine nursing care, diagnose and manage acute and chronic conditions, review patient medications, and offer guidance on medication usage.

• Coordination and Record Keeping:

• Coordinate the provision of travel medicine services, request pathology services, maintain accurate clinical records, and ensure compliance with practice policies, including Snomed codes.

Qualifications:

• Qualified Registered Nurse with NMC Pin

• Experience working within Primary Care, with desirable GP surgery experience

• All up-to-date training certificates on standard Practice Nurse Duties

• Enhanced and in-date DBS.
