A carpenter by trade, he spends time in his workshop, enjoys movies, playing cards, gardening, going to the gym, swimming, and painting. Jack sustained a Traumatic Brain Injury (TBI) and complex orthopedic injuries six years ago; he is a full-time wheelchair user. He can become fatigued, struggles with short-term memory, and may be impulsive. He requires support with transfers, either with another team member or using a transfer aid. Jack communicates verbally and is dedicated to his rehabilitation; he has detailed care plans followed by all team members. Jack is also a Type 1 Diabetic.
Overview of role: The team provides 2:1 support most days, depending on activities and plans, and 1:1 care at night. Jack's weekly routine is planned with his wife and shared with the team.
