Remuneration:Circa £125,000

Description:Part OR Full time Performer Dentist required for 2-5 days (flexible) in a busy, mixed, well established INDEPENDENTLY OWNED practice near Boston, Lincolnshire
• Available immediately or notice periods taken into consideration.
• up to £15,000 (pro-rata) Golden Hello subject to NHS criteria*
• Independent practice.
• Full time position, Monday to Friday.
• Part time hours also considered.
• Practice hours are Monday, Wednesday and Friday 8am-5pm, Tuesday 8am-6pm, Thursday 8am-7pm and Saturday 10am-4pm.
• Flexible UDA allocation.
• From £13.50 per UDA (rate and UDA target negotiable)
• Good private potential.
• All skill sets welcome and clinical freedom in practice.
• Existing patient list.
• Computerised (SFD), Digital X-Rays.
• Available now, start date negotiable

- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
