Role Summary
To assess, provide and review nursing care, advice and information to patients and carers within the home environment or alternative care setting. Aim to maintain patients in their own preferred place of care, enabling them to maximise their independence and quality of life. Work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Manager, using their skills as a Registered Nurse. May hold a designated caseload and deputise in the absence of the Nurse Specialist (Community).

We are seeking a Fixed Term Band 5 Registered Nurse to join our Community Nursing teams in Seaton, Axminster, and Sidmouth to cover a 1-year maternity leave.
Located in East Devon, our team is busy, dynamic, and forward-thinking. We pride ourselves on being friendly and dedicated to providing high-quality care, mainly in patients' homes. We support patients to maximize independence and quality of life in their preferred place of care.
The role involves delivering clinical interventions such as end-of-life care, wound management, and long-term condition management. You will collaborate with social services, therapy colleagues, GPs, and other staff. Strong communication and assessment skills are essential for planning, implementing, and evaluating personalized, patient-centered care.
We offer high-quality training, ongoing professional development, supervision, and a tailored induction based on your experience. Candidates should have good skills, be comfortable with digital technology, flexible, adaptable, and willing to embrace change. A current driving license and access to transport are required, as the role involves working independently in various locations.
