Job Summary

We are seeking someone with experience in managing minor ailments, chronic disease, and prescribing. The sessions will include telephone consultations, home visits, and both acute and planned consultations.

Main Duties of the Job

The ANP will provide care for patients from initial history taking, clinical assessment, diagnosis, treatment, and care evaluation. This role uniquely combines advanced nursing skills with clinical diagnostic and treatment capabilities. The ANP will collaborate with the multidisciplinary general practice team to meet patient needs, supporting policy and procedural delivery.

Job Responsibilities

1. Manage a clinical workload in collaboration with GP colleagues based on the practice population.
2. Provide holistic healthcare, including early disease screening and acute problem diagnosis.
3. Demonstrate knowledge of acute and chronic conditions' pathophysiology.
4. Accept referrals from other disciplines independently.
5. Conduct autonomous assessments of undiagnosed problems considering physical, emotional, social, cultural, and mental health issues.
6. Use sound judgment to prioritize conflicting needs.
7. Formulate diagnoses through history, examination, and investigations.
8. Initiate diagnostic and therapeutic interventions, considering safety and efficacy.
9. Develop ongoing, evidence-based health plans with patients and families, emphasizing prevention.
10. Interpret diagnostic tests and initiate treatment in liaison with GPs.
11. Evaluate patient progress and modify care plans for optimal outcomes.
12. Prescribe independently within legal and policy frameworks.

Person Specification

Experience

• Essential: Experience in primary care.
• Desirable: Experience in chronic disease management.

Qualifications

• Essential: Qualified Nurse Prescriber, Postgraduate diploma or degree.
• Desirable: Qualified Triage Nurse, Minor Illness Qualification.
