Job summary

• You will be working from the NHS Health Check delivery location with some very occasional working from home or other council base. This role is to cover the Somerset area.
• Practically you will need to be able to travel in a timely manner to a range of delivery sites including transporting some equipment.
• This role will specifically suit part-time staff, as it's a role to cover 4.6 days a week. We can be very flexible on working hours and pattern and can consider options within school day / termtime only / evenings and or weekends and for part time posts from 14.8 hours per week or monthly equivalent.
• Pro rata 25 days annual leave (plus bank holidays), raising to 28 days after 5 years continuous service.
• Fixed term contract until 31 March 2026.

Main duties of the job

We're working to improve the lives of people in Somerset through earlier detection of cardiovascular disease and promotion of healthier lifestyles in Somerset and you'll be a key part of that. NHS Health checks are part of the broader healthier lifestyle service for public health in Somerset. Your day-to-day work will involve:

• Delivering NHS Health Checks across a range of community locations like workplaces, libraries or other public settings.
• Taking and assessing the results from physical measurements, lifestyle questionnaires, point of care blood tests, and a cardiovascular risk assessment.
• Motivating individuals to make and maintain positive health lifestyles.

We offer ongoing support, training and guidance on how to deliver NHS Health Checks to help you be the best you can be. But it will really help if you:

• Have a basic understanding of the impact of lifestyle factors on health and wellbeing.
• Are aware of public health functions and the role of public health within local authorities.
• Are happy to travel to sites and work independently with attention to detail and accurate recording.
• Have excellent interpersonal skills influencing, negotiating and communication (written and verbal) skills, and able to be sensitive and empathetic.

Work as part of a team to deliver the NHS Health Checks service in areas of identified need across Somerset. Travelling independently to community settings and workplaces to deliver NHS health check clinics to eligible individuals. Maintain accurate and timely records on the client database and input accurate data following each health check in line with local protocol.

Maintaining a professional, person-centred approach to delivering health checks and communicating the results of a health check to individuals.

Take key measurements included in a health check such as blood pressure, cholesterol, height, weight, and glucose levels.

Attend relevant public health training for safe, effective and evidenced-based delivery of NHS Health checks. As well as healthy conversations training including Making Every Contact Count (MECC).

Promote and signpost to local support services as appropriate following a completed health check.

Use own initiative to maintain quality of delivery for services and escalate concerns as needed for more complex or strategic problem solving.

Monitor usage and wastage of stock ensuring accurate records are maintained and shared for accurate re-ordering.

Ensure appropriate disposal of any clinical waste related to own work areas following local protocol.

Appropriate use of equipment to record and perform measures in the NHS Health Check including Point of Care blood testing equipment for the assessment of blood lipid profile and sugar levels.

Conduct and report results of EQA (External Quality Assessments) and IQC (Internal Quality Control) of Point of Care testing equipment.

Undertake a risk assessment prior to the delivery of health checks in a new setting.

Participate in thedissemination of good practice to stakeholders and other professionals as relevant.

Promote public health activities where appropriate.

Work closely with public health teams to understand their local offer and promote opportunities to support public health messages where appropriate.
