An exciting opportunity has arisen for experienced HCSWs to join our Intermediate Care Team based in St Andrews.
The Intermediate Care Team provides services to the adult population within the St Andrews locality, delivered either at service users' homes or as out-patient care when necessary.
The primary role involves working with Occupational Therapy and Physiotherapy staff to deliver therapeutic treatment programmes, aiming to prevent hospital admissions and support early discharge. You will manage your own caseload, conducting assessments, progress reviews, and discharges within your scope of practice.
Candidates should have an HNC/HND in Health and Social Care or equivalent, or SCQF level 8 qualification such as Postural Stability Instructor. Experience as a therapy support worker and working with older adults are essential.
Excellent organisational and communication skills, the ability to work independently and as part of a multidisciplinary team, and flexibility to work across different community hospital sites are required. Public holiday working is expected.
We offer regular supervision, personal development planning, and training opportunities. The role requires the ability to travel throughout the area.
Interviews will be conducted in person.
