What you will do:

1. Collaborate with a multi-disciplinary team to deliver holistic care assessments, promoting independence and wellbeing.
2. Guide adults through reablement interventions, coordinating services and providing support within a client-led care plan.
3. Carry out comprehensive assessments including Mental Capacity Assessments following the Mental Capacity Act 2005.
4. Implement the Care Act 2014 in your practice, ensuring client welfare and safeguarding vulnerable adults.
5. Manage a diverse caseload, providing targeted advice and consultancy to peers and other stakeholders.
6. Maintain and update records as per Council Policy, ensuring accuracy and compliance with data protection legislation.
7. Actively participate in multi-disciplinary meetings, enhancing team collaboration and service delivery.
