Band 6 Home Treatment Team Practitioner

Base: Theta Building, Frimley

Hours: 37.5 hours a week

Job Overview

We are excited to share an opportunity for a Band 6 Community Mental Health Nurse. This role involves working within one of our hubs located in Frimley, providing crisis and home treatment services for emergency, urgent, and routine referrals in the Surrey Heath and Northeast Hampshire area. As a Band 6, you will work alongside both junior and senior practitioners, taking a lead in coordinating shifts, delegating tasks, and liaising with key stakeholders. You will be required to respond urgently to assessments and treatments for individuals experiencing a mental health crisis. The role requires both autonomous working and effective team collaboration.

This integrated role supports triage, rapid response, and home treatment components of the service. The post holder will be responsible for creating a safe, supportive, and accessible environment with a no wrong door approach.

Main Duties of the Job
  • Embodies the visions and values of Surrey & Borders Partnership Foundation NHS Trust
  • Adheres to the code of practice/conduct of your registering body at all times
  • Discharges professional responsibilities under the Mental Health Act (1983), the revised Mental Capacity Act (2005), and the NMC Professional Code of Conduct
  • Is accountable and responsible for own practice
  • Develops, implements, and evaluates evidence-based care programmes, setting standards and liaising with other agencies, especially regarding discharged patients under the Care Programme Approach
  • Participates in multidisciplinary assessments of individuals with mental health issues or vulnerabilities in the community, providing a mental health perspective and expertise

As this role is community-based, travel between multiple sites and trust locations within set working hours is required.
