What you will do:

1. Provide expert physiotherapy assessment and intervention to facilitate patient discharge
2. Develop and implement personalised care plans focusing on safe and effective discharge strategies
3. Collaborate closely with a multidisciplinary team to ensure comprehensive care
4. Maintain accurate and up-to-date patient records, adhering to professional standards
5. Offer guidance and support to patients and their families during the discharge process
6. Assess and address potential barriers to discharge, making recommendations for additional support when necessary

As this role requires a registered professional, you must be HCPC registered and hold a full driving licence to visit diverse patient locations.
