Job summary
This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via a separate process.
An exciting and rewarding opportunity has arisen for a Band 5 Learning Disability Nurse/Care Co-ordinator working within an established Integrated Team alongside an MDT including Social Workers, Nursing, Psychiatry, Psychology, Physiotherapy, Occupational Therapy, Speech and Language Therapy, and administrative staff.
Durham County Council leads the Integrated Learning Disability Teams. Candidates will primarily use the Durham County Council system Azeus for recording, with access to TEWV systems, training, and NHS mail.
The team collaborates to ensure high-quality care, supporting individuals with learning disabilities and their families, utilizing the Care Act and TEWV clinical pathways.
The service hours are 9am to 5pm, Monday to Friday. The post also involves being part of the Nurse on Call rota, covering out-of-hours from 8pm to 8am.
Main duties of the job
The successful candidate should have a genuine desire to improve mental health and well-being for adults with learning disabilities and challenging behaviors. They will work within a multidisciplinary team, providing high-quality secondary care, using trauma-informed and recovery-based approaches.
Applicants must be able to travel independently across the locality with access to a suitable vehicle, with accommodations under the Equality Act 2010.
