Main duties of the job

The team respond to both planned and unplanned nursing needs such as palliative care, catheter care, and supportive nursing visits to monitor patient stability and provide support to both formal and informal carers. The aim of the service is to deliver high-quality nursing care during out-of-hours periods and reduce unnecessary hospital admissions.
