Job Description: Social Worker - Eating Disorders Inpatient Unit
The Eating Disorders inpatient unit at St Ann's Hospital provides comprehensive specialist inpatient services to north central and north east London. It is also a national provider of specialist eating disorder inpatient treatment. The unit primarily serves patients over 18 years old with a diagnosis of anorexia nervosa.
The post holder will:
- Lead in identifying social care needs upon admission and incorporating them into the care plan.
- Provide training, support, and consultation to colleagues.
- Coordinate services to meet social care needs upon discharge.
- Liaise with secondary care teams responsible for social care discharge planning to prevent delayed discharges.
- Lead on the Transforming Care agenda for patients with comorbid learning disabilities.
Responsibilities include:
- Conduct triage and comprehensive social work assessments to inform care recommendations.
- Prepare accurate reports aligned with quality standards.
- Make dynamic social care judgments, managing complex needs with high interpersonal skills.
- Facilitate referrals and interventions with relevant agencies, utilizing local service knowledge.
- Represent the team in strategic management of high-risk patients through clinical reviews and case conferences.
- Oversee patient care, involving carers and resources, supporting multidisciplinary care plans and risk management.
- Maintain professional standards in therapeutic interventions, including individual and group therapies.
- Plan, implement, and evaluate social work interventions considering patient and community risks.
- Provide specialist consultations, liaise with health and social service providers, and maintain confidential records.
- Make referrals, coordinate with inpatient/outpatient services, and support clients with local mental health teams.
- Promote mental health awareness, community initiatives, and ensure culturally sensitive services, offering evidence-based advice.
